You can prepare one survey which assesses your student's learning interests, learning styles and levels of readiness or you can prepare different surveys.

Plan the content according to level of readiness where you can differentiate basic, grade level and higher level objectives;
Plan a lesson according to student learning profile where you can differentiate the tools used during instruction and in the presentation of projects;
Plan a lesson according to student interest where you use materials that will appeal to visual, auditory and kinesthetic learners.
